TERMS & CONDITIONS

I agree that my decision to participate in Womb Remembrance (henceforth referred to as the “Workshop”) with Sarah Campbell & Adelina Vaughn (henceforth referred to as the "Facilitators") is a personal decision. No one has made any promises or warranties as to the results or benefits I will receive or as to any specific results I will realize from my participation in this Workshop. I understand that this Workshop is not medical advice or council, therapy or psychological counseling and is not a substitute for the treatments or services ordinarily provided by health care professionals for physiological or psychological complaints. If I desire medical advice, therapy or psychological counseling, I will seek it from a licensed provider. I understand that the Workshop is for my personal education and benefit only and may not be appropriate for others. I understand that information shared in the Workshop is strictly confidential and can not be given out unless I sign a release form.

I understand that all materials used in this Workshop are confidential and proprietary in nature and other than for my own personal use, I will have no future right to these materials or processes in any way without express training, written permission, and licensing by the Facilitators. I accept full responsibility for any product or material loaned to me as part of the participation in this Workshop and commit to return the same in good working order.

I affirm that I have answered all questions on this form honestly and fully disclosed all the information requested. I assume all risk associated with participating in this Workshop and release the Facilitators and any other person associated with the aforementioned persons and entities (collectively, the “Released Parties”) from, and shall defend, and indemnify, and hold them harmless from and against, all liabilities, claims, actions, losses, causes of action, and costs in any way arising out of my participation in the Workshop. I release the Facilitator and Released Parties from any liability for injuries due to their negligence occurring now or in the future, during or after my participation in this Workshop.

REFUND POLICY:

There are no refunds offered for Womb Remembrance programs.

I CONFIRM THAT I HAVE READ AND UNDERSTAND THIS AGREEMENT FULLY AND INTEND TO BE LEGALLY BOUND BY IT.
